What is a DD214 Service 2 form? ›

Copies 2 and 4 are identical long-form copies of the DD-214 and contain all the necessary information to show proof of military service and honorable discharge. If a veteran needs to obtain a long-form copy of their DD-214, they will receive a Service 2 copy.

DD Form 214 / DD214 / DD 214 Discharge Papers and Separation Documents. A Report of Separation is generally issued when a service member performs active duty or at least 90 consecutive days of active duty training.

DD214s are issued with multiple copies, each going to different people and government agencies. The veteran receives copy 1 automatically and copy 4 if he requests it. Copy 1 has blocks 1-22 filled out—copy 4 has blocks 1-22 and additional blocks 23-30.

In some cases, the information on your DD214 form may be incorrect. If errors are general and clerical in nature — a name is spelled wrong, some numbers are transposed — you can ask for a correction to the document. The government issues a corrected form, which is called the DD215.

The DD 256 and 257 are issued when the person has not met the active duty requirements to be considered a veteran by the DoD. But having a DD214 form doesn't automatically mean you are a veteran! What is truly bothersome is that people who have served but don't qualify as a veteran can request a DD214.

Service members are given the option of accepting the "short form" edited Member 1, "long form" unedited Member 4 or both copies upon separation. The most important copy of the DD 214 for the individual is the long form copy.

There are only six types, making it easier for everyone to understand. These are (1) “Honorable” or “Under honorable conditions,” (2) “General under Honorable Conditions,” (3) “Other than Honorable” (OTH), (4) “Bad Conduct” (BCD), (5) “Dishonorable” (DD), (6) “Entry Level (ELS) or Non-Characterized.”

A reserve member is given a DD214 only if they were once on active duty. This file is issued after a person has undergone initial training and is separated from active duty after serving for at least 90 consecutive days. If that person was a reserve member but never activated, they won't receive a DD Form 214.
